Please answer each question accurately to ensure proper fulfillment and delivery of your course completion history. Please also check that your Genius student account information is accurate and entirely completed.

Grades for courses you are currently enrolled in may not appear on your course record. Please wait to submit your request until after courses are completed if you would like for them to be included on your course record.
